This white paper addresses the challenge of integrating traditional, complementary, and integrative medicine (TCIM) into modern electronic health record (EHR) systems. While current EHRs support biomedical interoperability, they often lack structured, machine-interpretable mechanisms for capturing TCIM diagnostic frameworks, interventions, and therapies. Building on IEEE Industry Connections definitions of EMRs and EHRs, the paper highlights how TCIM data is frequently documented as unstructured or semantically reduced information, limiting interoperability, computability, and safety monitoring.

In alignment with the WHO Global Traditional Medicine Strategy 2025–2034 and growing AI adoption in healthcare, the paper proposes a layered semantic interoperability architecture for FHIR-based EHR ecosystems. The approach enables structured coexistence of biomedical and TCIM ontologies through domain context metadata, intervention parameterization, practitioner profiling, and AI-assisted terminology harmonization. The objective is to improve multimodal care documentation, longitudinal data completeness, and interoperability while preserving ontological integrity and compatibility with established healthcare standards.
